Nursing Home Transition and Diversion Waiver Program:
A Template for Developing Policies & Procedures that are consistent with the requirements of the NHTD Program.

From 2009-2010 current NYSADSA Board Members created a guideline for the policies and procedures of Adult Day Services programs. We have done all of the work for you, and this is a wonderful opportunity for current NYSADSA Members and other professionals, to participate in the Nursing Home Transition and Diversion Waiver Program (NHTD). All New York State Department of Health guidelines for program application are included. The document is user-friendly and allows you to complete it easily, and according to your agency's characteristics.   Costs are: 

  • $75 for NHTD "Work Group" NYSADSA Members
  • $100 for Current NYSADSA Members
  • $350 for Non-NYSADSA Members.

DETERMINING UNIT COST FOR ADULT DAY SERVICES

To calculate unit cost for Adult Day Services, a program determines all expenses for the past 12 months or a calendar year.  These costs should include direct and indirect service costs and all administrative/operational overhead costs.  Values should be included for in-kind support and volunteer utilization.  The total amount of annual expenses is used to calculate a variety of unit costs, including hourly or daily participant unit costs.

 Determine Unit Cost per Program Day: divide annual expenses for an adult day care program by the number of program days in the year.  For example if a program's annual expenses are $150,000 and they are open for 250 days per year, the cost per program day is $600.

 Determine Unit Cost per Program Hour:  divide the cost per program day by the number of hours the program is open.  For example, if the program is open for 8 hours, the cost per program hour is $75.

 Determine Unit Cost per Participant per Hour: divide the cost per program hour by the average daily attendance.  For example, if the program averages 10 participants per day, the unit cost per participant per hour is $7.50.

 or

 Determine Unit Cost per Participant per Day: divide the cost per program day by the average daily attendance to get the unit cost per participant per day.  For example, if the cost per program day is $600 divided by an average daily attendance of 10, the unit cost per participant per day is $60.
