Social Adult Day Services Information
The New York State Office for the Aging (NYSOFA) contracts with the New York State Adult Day Services Association to provide technical assistance to managed care organizations, providers of social adult day services and those interested in starting an adult day services program.
NYSOFA and the Area Agencies on Aging (AAAs- located in counties) do not provide technical assistance to individuals or groups who want to start a social adult day services program. Please contact Victoria Palasieski, NYSADSA Administrator at 518-694-5366 or NYSADSA@caphill.com.

The Conversation Project and the Institute for Healthcare Improvement have collaborated to create Your Conversation Starter Kit (For Families and Loved Ones of People with Alzheimer’s Disease or Other Forms of Dementia.)
This booklet is specifically designed to guide families and loved ones of people with Alzheimer’s Disease or another form of dementia regarding their conversations with their family members and loved ones about their end-of-life wishes and care. It includes both guidance and fill-in-the-blank questions in an easy-to-use workbook format. To be most effective, these issues should also be discussed with appropriate medical providers.
